Affidavit of service of notice on contributory I, ................ (name and description of deponent), of ................, solemnly affirm and say as follows: 1. I did on the ................ day of ................ 19 ........, in the manner hereinafter mentioned serve a true copy of the notice now produced and shown to me and marked 'A' upon each of the respective persons whose names, descriptions and address appear in the second and third columns of the First Schedule to the list of contributories of the said company settled by the Official Liquidator of the said company on the ................ day of ................ 19 ........, and now on the file of the proceedings of the said company. In the tabular form at the foot of such copies respectively, I inserted the number on list, name, description, address, in what character included, the number of shares [or extent of interest] and the amount called up and the amount paid up in respect of the shares [or interest], of the person on whom such copy of the said notice was served, in the same words and figures as the same particulars are set forth in the said Schedule. 2. I served the said respective copies of the said notice duly addressed to such persons respectively according to their respective names and addresses appearing in the said Schedule by registered post on ................. at the post office at ................. and the postal receipts and acknowledgements received from the said post office in respect thereof are annexed hereto.
