(name and description) of, etc., solemnly affirm and say as follows: 1. That I did on the .......... day of .......... 19 ....., send to each creditor mentioned in the company's statement of affairs [and to each contributory mentioned in the Register of Members of the company] a notice of the time and place of the (respective) meeting(s) of creditors (and contributories) in the form(s) annexed hereto and marked 'A'1 (and 'B')2 2. That the notices for creditors were addressed to the said creditors respectively according to their respective names and addresses appearing either in their proofs or in the statement of affairs or in the books of the company, or to the last known addresses of such creditors [and that the notices for Contributories were addressed to the contributories respectively according to their respective names and addresses appearing in the Register of the company or to their last known addresses.] 3.
