G.S.R. 787(E), DATED 14-11-2008 New Delhi, the 14 th November, 2008 Subject:- Investor Education and Protection Fund (Awareness and Protection of Investors) (Amendment) Rules, 2008 - Substitution of Form 1 G.S.R. 787(E).- In exercise of the powers conferred by sub-section (1) of section 642 read with section 610B of the Companies Act, 1956 and sub-section (3) of section 205C of the Companies Act (1 of 1956), the Central Government hereby makes the following rules to amend the Investor Education and Protection Fund (Awareness and Protection of Investors) Rules, 2001, namely : 1. (1) These rules may be called the Investor Education and Protection Fund (Awareness and Protection of Investors) (Amendment) Rules, 2008. (2) These rules shall come into force from 7-12-2008. 2. In the Investor Education and Protection Fund (Awareness and Protection of Investors) Rules, 2001, for the Form 1, prescribed under rule 3, the following form shall be substituted, namely: FORM 1 Statement of amounts credited to investor education and protection fund [Pursuant to rule 3 of the Investor Education and Protection Fund (Awareness and Protection of Investors) Rules, 2001] Note - All fields marked in* are to be mandatorily filled. 1. ( a ) *Corporate identity number (CIN) of company Pre-fill ( b ) Global location number (GLN) of company 2. ( a ) Name of the company ( b ) Address of the registered office of the company ( c ) *e-mail ID of the company 3. *Service request number (SRN) in respect of payment made to the fund Pre-fill 4. *Date of payment of amount to the fund (DD/MM/YYYY) 5. *Amount credited to the fund 6. *Mode of payment Challan payment (cash, cheque, demand draft) Online payment 7.
