Due date of filing ST-3, Service Tax

Dear Sir,

Can anybody mention the due date of filing ST-3 Returns w.e.f. April 2011 to till Sept.2015 as there were amendment of due date ( extension of last date ) by many a times since the introduction of e-filling.

There were few extensions of due dates and relevant notifications were also issued.
