Amount of Form C, VAT + CST

What should be amount of Invoice on Form C ? Should it Cost +  Excise + CST or  Cost + Exice ?

The amount would be the total amount but net of CST amount. In other words, cost + excise duty + Edn. Cess + SHE Cess.
