Pursuant to the Companies Act, 1956, and the Rules, made thereunder I, ........... the undersigned, the Official Liquidator of the above-named company, hereby certify that since settling the list of contributories herein dated the ................. day of ................. 19 ....... I have settled the following supplemental list of contributories of the said company consisting of persons who are holders of shares in [or members of] the said company and who are contributories of the said company. 2. The said supplemental list contains the names of such persons together with their respective addresses and the number of shares [or extent of interest and the amount called up at the commencement of the winding-up and the amount paid up at such date in respect of the shares [or interest] to be attributed to each. 3. In the first part of the said list such of the said persons as are contributories in their own right are set forth. 4. In the second part of the said list, such of the said persons as are contributories as being representatives of or being liable for the debts of others, are set forth.
