Dear mr. M S rajpurohit,

1.  You may submit the affidavit  as per Appendix 24  of the Handbook of procedures

2.  Before going for affidavit, you have to file a complaint to the jurisdictional police station. Obtain acknowledgement / FIR copy

3.  Have the affidavit printed on a stamp paper as per your state laws.  You may go for Rs. 200 rupee stamp paper to be on  the safer side

4.  Have the affidavit  notarized

5.  prepare a covering letter explaining the reasons for seeking duplicate copy of EPCG licence

6.  Enclose, affidavit, ack.  and the FIR copy

7.  Follow up to get the duplicate EPCG copy

8. Hope,  this is sufficient for you

With best wishes and regards,

Rangaswamy

Please check :

Whether imports are completed or not ?

If imports are already completed, file FIR and get utilisation certificate from customs.

No need to get duplicate. Issue of duplicate is costly affiar.

For fees for issue of duplicate, it is 1% of CIF value/duty saved unutilised.

Also, it is better to apply for new licence for imports not made, the cost of application fees for new lice, will not exceed Rs.50,000/-
